Covid 19 Wage Subsidy Consent to use Private Information

The Ministry of Social Development will use the information provided in this application to assess your eligibility to receive the COVID-19 Wage Subsidy.

I consent to,

1. The information about me being provided to the Ministry of Social Development (MSD) with respect to the subsidy application (both at the time of application, and any information provided at a later time):
   
   a) being used by the MSD to make decisions about the application, and to audit and review any subsidy that is granted (to applicants) and how any subsidy granted is paid to employees; and
   
   b) being shared by the MSD other agencies (including non-government agencies) to the extent necessary in order for the MSD and its auditors to make decisions about the application, and to audit and review any subsidy that is granted (to applicants) and how any subsidy granted is paid to employees; and
   
   c) being used by the MSD to make decisions about other assistance and entitlements to the extent the application and any subsidy granted is relevant to me (for instance, where ANZCO’s application is relevant to my application for other assistance).
2. Other agencies (including non-government agencies) providing information about me to the MSD and its auditors, to the extent necessary in order for the MSD to make decisions about ANZCO’s application, and to audit and review any subsidy that is granted (to applicants) and how any subsidy granted is paid to employees.
